ACCIDENT:
REQUIRED DOCUMENTS IN THE CASE OF CAR INSURANCE CLAIM FILES
Traffic accident report/agreement protocol or record of statement and eyewitness
Statement in case there is not any police report or record

IN CASE THE VEHICLE IS STOLEN:
Police station application record
Original of vehicle license bearing stolen seal
Positive D/M consent if there is a pledge on the vehicle and notarized letter stating that pledge annotation is removed
Receipt of discharge issued by Ministry of Finance
Letter issued by traffic registration department that traffic registration is erased
Police station and auto theft office letters stating that the vehicle couldn’t be found
Power of attorney (a copy of it to be received from our company)
Original and spare keys

1- Material Damages
A) Vehicle Claims
Officially certified copy of Traffic Accident Report, or any accident report prepared between the parties and, records of statements or eyewitnesses, if any;
License of the vehicle of injured party;
Pictures of damaged vehicle, if any; and
Bank account details (bank - branch name, IBAN number) of the owner of the damaged vehicle / damaged third person.
B) Loss of Value
(A) Documents requested by Vehicle Claims department:
Statement of request for loss of value; and
Adjuster report if a survey was made for material damage.
C) Other Claims (for properties of third person)
(A) Documents requested by Vehicle Claims department:
Certificate of ownership of damaged property (title deed, invoice for purchase, official letter of public organization, etc.),
Invoice and accounting records for repairing the damaged property of third person.

Physical Injuries
1. Permanent Disability
Medical board report prepared according to the Regulation on Assessment of Disability for Adults and Regulation on Assessment of Special Needs for Children;
Epicrisis report;
General forensic examination report;
Reports on all medical examinations and treatments;
Identity photocopy of the injured party;
Accident report, expert report (if any), estimation report or court decree;
Final income status document of the injured party as of accident date;
Bank account details (bank - branch name, IBAN number) of the beneficiary; and
Statement of explicit consent provided by the injured party for accessing, processing and transferring the health data.
2. Death
Accident Report, Expert Report (if any), estimation report or court decree;
Certificate of inheritance;
Certificate of death and autopsy report of deceased;
Current certificate of identity register copy indicating the details of parents;
Final income status document of the deceased person as of accident date; and
Bank account details (bank - branch name, IBAN number) of the beneficiary.
3. Personal Data Verification Documents
For Natural Persons: Photocopy of the birth certificate and declaration of signature, contact details.
For Legal Entities: Photocopies of tax statement, trade registry gazette, signature circular and birth certificates of authorized persons indicated in the circular.

Providing information like policy number, contact numbers, address details, date of damage, damaged value, reason of damage, estimated claim amount, etc., when submitting the Claim Report will speed up survey and file tracking processes.
While the specific steps and necessary paperwork may differ based on the type and extent of harm, the following are typically the documents that will be asked for.
Theft
If there is a theft claim, you need to file an application with the appropriate Police or Gendarmerie Station and report the theft that took place on the insured property or for the insured belongings.
Required Documents for the Theft Claims:
Eyewitness report;
Record of statement;
Letter of police/gendarmerie station stating that the offender and stolen properties are found or not (after a waiting period of 1 month);
Explanation detailing how the damage happened;
Letter of request for indemnity explaining the loss;
Photocopy of title deed or rental contract;
Copies of invoices for stolen goods (if any).
Fire
Fire brigade report (certified copy);
Records prepared by official authorities;
Invoices for repair, if any;
Copy of land title or rental contract in the case of building claims;
Verdict of non-prosecution received from Public Prosecutor’s Office (to determine whether it is deliberate or not);
Photos indicating the damaged parts;
Letter of request explaining the loss;
Detailed statement in case the fire was extinguished by the insured party before it grows;
If the damage was caused by a third person, the name and address of this person, if possible.
Earthquake
Letter of request stating the loss in your damaged properties (building and/or household goods);
Certified report issued by the Department of Disaster Affairs indicating the condition of damage of the covered asset;
Invoices for repair (if any);
Copy of title deed or rental contract;
Property tax return and/or occupancy permit;
Photos of any damaged sections;
Copy of mandatory earthquake insurance policy.
Glass Breakage
Explanation detailing how the damage happened;
Request letter for compensation detailing the damage incurred;
Photos indicating the damaged glass;

Internal Water, Flood, Flash Flood

REQUIRED DOCUMENTS IN THE CASE OF MARITIME TRANSPORTATION
Original or photocopy of invoice
Original or photocopy of bill of lading
Vessel reserves record, if the goods are unloaded as damaged from the vessel (this document must certainly bear the signature of vessel captain or agency)
Inspection record
Customs reserve record, if the commodity was damaged within customs zone
If the goods became damaged within customs zone and the customs administration avoids from providing the reserve record, the goods must not be withdrawn from the customs and a court determination must be requested by applying to the local Civil and Commercial Courts of First Instance (judicial expenses for such determination will be borne by the insurer)
Freight agreement, if the transportation was awarded by an agreement
Cargo report (if the vessel reserve record is already obtained, there is no need for this document)
Name/title and address of shipowner and their agency in Türkiye (important)
Timesheet and packing list
Customs closing declaration
Tallies and bills of weight in case of evacuation free alongside ship
The detached delivery order or non-delivery order, if the goods were unloaded as missing or were never unloaded from the vessel
Report of Lloyd’s adjuster in the case of damages in abroad
Loading manifest
Loading and discharge port Ullage reports (supervision reports), preparation letter, empty tank certificate, protest letters in the case of transportation of liquid commodities

REQUIRED DOCUMENTS FOR THE LAND-AIR-RAILWAY TRANSPORTATION
Invoice, waybill or bill of lading (shipment waybill in case of domestic transportation)
If the goods are unloaded as damaged from the vehicle, the reserve record (this document must bear the signatures of relevant persons transporting the commodity, the truck driver, and railway administration)
Inspection record
Photograph
If the goods are damaged within the customs zone, the customs reserve record (this document must bear the signatures of customs officers or operators)
If the goods are damaged within customs zone and the customs administration avoids from providing the reserve record, a court’s determination is required.

Customs closing declaration
Freight agreement, if the transportation was awarded by an agreement
Photocopies of the traffic accident report, the driver’s license and vehicle license, if the damage was occurred as a result of accident
Name-title and address of the transporting company and name-title and address of their agency in Türkiye
Truck scorecard (for transportations in abroad)
In case of transportation by any means of transport, if no damage is detected at the time of delivery from the vehicle or at first glance or if the damage is detected later, it must be reported in writing to the carrier or its representative (agency) at the destination port within 3 days at the latest from the date of delivery.
